When to Refer

Should you find your patient is highly esthetically driven, congenitally missing several or all of his or her teeth, has a complicated occlusal discrepancy, severe erosion, abrasion or attrition, had unsuccessful previous treatment, difficulty in recording an accurate bite, requires construction — or you'd prefer not to have the burden of responsibility for their complex situation — we are here for you.

Please complete our secure online referral form or print a PDF referral form to mail or email including any applicable x-rays and recent FMX. You can email referrals to info@prosthodonticsassociates.com.

Hours & Availability

Our regular hours of operation are 8:00 AM to 5:00 PM, Monday through Thursday and 8:00 AM to 4:00 PM on Friday. To assist patients in the event of an emergency, we provide a doctor on call 24 hours a day, seven days a week, 365 days a year.
